Landmark Home Health

• Dusting

• Vacuuming

• Sweep and mopping

• General cleaning of the consumer’s
  living room, dining room, kitchen,
  bathroom and bedroom.

• Taking out trash

• Doing laundry

• Light food shopping

• Picking up medications

 

Personal Care

• Assistance with bathing/showering

• Help into/out of the tub/shower

• Assistance with shampooing hair

• Assistance with shaving

• Assistance with incontinence care

• Medication reminders (Personal Care Workers are NOT permitted to administer medications)
   

Options Program for individuals aged 60 or older

• The program is funded through the Area Agency on Aging in both Allegheny and Westmoreland Counties

• Eligibility is determined by the Area Agency on the Aging

• The services provide the consumers assistance with light homemaking and/
  or personal

 

HSDF Program for individuals aged 18 to 59

• This program is funded through a grant from Allegheny County known as the  Human Services Development Fund

• These services provide the consumers assistance with light homemaking and/or personal care

• To be eligible for this program an individual must live in Allegheny County, have a disability plus either Medicaid or meet the income guidelines.  The Federal Poverty Guidelines are followed at 125% of the federal poverty level

 

Private Pay

• If an individual does not qualify for any of the above programs or may only need short-term help, Private Pay services are available

• Private pay services are available between the hours of 7 am – 5 pm, Monday through Friday, call for hourly rates

• 3 hour minimum must be prepaid
